How CCOR can help

CCOR is a New York State Licensed Home Care Service or LHCSA. With this license, CCOR can provide Home Health Aides (HHA), Licensed Nurse Practitioners (LPN), Registered Nurses (RN), Personal Care Aides (PCA) to work and oversee medical treatment in your home. We will work with you, your loved one and physician to design a care plan that keeps you independent in your home.

Our Care Managers can help individuals looking for support for themselves or loved ones. Find out what options are available to them. We are here to review what your insurance covers, what insurance options are available to you and how to get the most out of your benefits. Our Care Managers who specialize in home health services will help you get information to navigate the health care System.

TBI and NHTD Waiver Program

We are an approved provider of the New York State Department of Health Home and Community Based Services (HCBS). This includes Nursing Home Transition and Diversion (NHTD), Traumatic Brain Injury (TBI) and Medicaid waiver services.

Consumer Directed Personal Assistance Program

The Consumer Directed Personal Assistance Program (CDPAP) is a statewide Medicaid program that provides an alternative way of receiving home care services, where the consumer has more control over who provides their care and how it is provided. This allows the consumer to handle the responsibilities that are normally handled by the traditional home care agencies.
